Being better informed means making better decisions.
Our mission is to empower you with the best information and tools to make the right decision for your loved one. We want you to know that you’re in control and have options available to you.
What types of care do you offer?
We have a wide range of care professionals to fit the individualized needs of you and your family. Each one of them has a unique skill set and with the help of Nicole, we will find the right fit for you. Rest assured we will have a great team behind you that strives to meet the needs of the care while supporting and honoring each other with grace, a lot of laughter, and compassion.
Transitional & Respite Care
Transitional care applies to those coming home from the hospital or a rehab facility that still need continued care from a professional. We work closely with the doctors to have the client released to go home. Also, with this type of care we coordinate with PT and OT for continued rehab within the home. While respite care can be set up for those families who are caretakers but need a much needed break.
Palliative Care
Palliative care is meant to enhance an individual’s current care by focusing on the quality of life for them and their loved ones. Some examples of what we work with are memory care, cancer, heart disease, stroke, and Parkinson’s, just to name a few. Our teams work with the client to establish safety in the home, continuing comfort care and overseeing doctor visits. We work very closely with our clients and their family to establish as much independence in the home as possible, while still having a care professional present.
Hospice Care
We focus on care, dignity, comfort and quality of life during this time. For families approaching end-of-life, our team would help the terminally ill by reducing pain and addressing any needs. We also offer physical, psychological, spiritual and social needs of the client and their family. With our 24 hour service we can provide comfort day or night.
Death Doula
A death doula’s work ranges from logistical planning for the time before, during and after of death; conducting rituals or comforting practices; helping the dying person reflect on their life and values; and explaining the bodily functions of dying to caregivers
When is it time to hire care?
We use the following benchmarks to consider hiring care and and help relieve some of the burden, however, it can vary person to person. We welcome any of your questions to help determine when care should be considered:
- Any time close family members are seeing physical and/or cognitive changes that affect mobility or concern of a fall or other injury.
- The inability to do routine daily tasks such as getting dressed, taking medication, showering, etc.
- When caring for your family member becomes too difficult or overwhelming to manage without support.
What hours are your staff available?
Our teams have the ability to provide around-the-clock care. We understand that each care continuum is unique in needs and requires a wide range of support. Let us help ease your mind with the confidence in our 24 hour service(s).
Does your team offer spiritual support?
We understand that the road to true healing comes through a holistic approach, including helping people deal with the fears they have surrounding their next journey. Whatever path your spiritual practice takes, we provide nonjudgmental and loving support to patients and their family members.
